An employee was changing the bearings in a ventilation system motor. After the motor was put back together, the employee reached behind the motor with his left hand to check the belt tension. While the employee was checking the belt tension, the wind blew through the air duct and caused the ventilation fan pulley to turn. The employee's left pinky and index fingers were pinched between the pulley and belt and amputated.
