On or about January 26, 2016, at 2:45 a.m., an employee was practicing cutting a 3/4-inch steel plate on a cutting table with an oxygen propane torch when his 100-percent cotton work shirt caught on fire, causing second degree burns over 12 percent of his chest and stomach. He was hospitalized.
