On 2/13/2016 at about 1:30 p.m. an employee who was cleaning around a floor grain auger with an open gate fell into a hole around the auger. The employee's leg was caught in the moving auger and the leg was amputated around the knee. The employee was hospitalized.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.
