An employee was working on a 12-foot A-frame ladder, attempting to thread a piece of pipe. While pushing the pipe to align it, he was thrown off balance and fell forward approximately 4 feet (from the 4th or 5th rung) to the concrete floor, suffering a head laceration, two broken wrists, dental trauma, and a bruised knee.
