Final narrative
An employee was part of a crew performing work during the temporary shutdown of a boiler. He was raising a wash beam from inside the boiler by using an electric powered winch. He was operating the pendant controls of the winch and raising the beam. While raising the beam, his right hand was on the cable of the winch. The spool of the winch caught his glove, causing his hand to became caught in the winch. He pulled his hand back, amputating his right ring fingertip just below the nailbed.
