On March 23, 2016, at approximately 1:30 a.m., an employee was cleaning out a vertical blender. When she opened a 10-inch x 10-inch door on the side of the blender, her right hand was caught between the side of the blender and the moving auger, amputating four fingers. The machine was energized at the time of the incident.
