On or about April 29, 2016, an employee was pressure washing the Fitz Mill machine. The employee used a chain hoist to lift the spout lid, and because the equipment was still running, the lid came back and struck employee. The employee suffered face fractures, a broken sternum, and cuts on the left wrist.
