Final narrative
An employee was approximately 8 feet high, standing on an extension ladder that was leaning on the exterior wall of a house. He reached out to the right and above his head to place a nail in the fascia board when the ladder slid to the right and down the wall surface. He fell down to surface level and landed on the ladder, breaking his left fibula, tearing ligament near his left ankle, and rupturing his right Achilles tendon. .
