When the employee placed a piece of metal into the shear, the weight difference between the heavy end and lighter end caused the piece of metal to be raised above the shear table. The shear cycle then activated via the foot pedal, and the hold-downs and guard lowered and forced the piece of metal down toward the shear table. The employee's right index fingertip was pinched and amputated between the metal and the shear table.
