On or about September 9, 2016, an employee was unrolling scrap film from one roll and placing it onto another roll when the edge of the roll slipped off the roller bearings. The employee and a coworker attempted to lift the roll approximately 1/16" back onto the roller bearings, but the roll shifted, pinching and amputating the meaty portion of the left middle finger between the roll and the cradle.
