On November 25, 2016, at 2:30 PM, an employee was watching a coworker set up a die in a full revolution mechanical power press when the employee's left hand entered the top ejection post that was attached to the platen. The coworker then jogged the machine and the top rod crushed four fingers on the employee's left hand. The middle and ring fingers had to be medically amputated below the first joint.
