On Wednesday, December 21, 2016, at approximately 2:30 PM, an employee was operating tube-straightening process equipment when a tube stopped in the process. He placed his left hand on the tube. As he did, the tube began rotating again. The motion caught the employee's left forearm, fracturing it and requiring hospitalization.
