An employee was operating a forklift with a dump hopper attached to the forks. He drove the forklift to a dumpster, exited the vehicle, and activated the hopper's rotation handle to dump a load of paper into the dumpster. His left arm was caught in the moving parts of the hopper, which resulted in a compound fracture between the elbow and the shoulder. He was hospitalized and had surgery.
