On June 8, 2017, an employee was inspecting the nailing of roof plywood and the strapping of the rafters. While walking back to the access point, the employee slipped and fell 13 feet to the ground, fracturing his left tibia and suffering a compression fracture to his L2 vertebra. Fall protection was not worn at the time.
