On June 10, 2017, an employee opened a tub of liquid chlorine bleach with pliers and rinsed the pliers in a stream of what appeared to be running water, but was actually disinfectant. The employee then experienced shortness of breath and lowered oxygen saturation, requiring hospitalization.
