An employee was hand feeding a plastic board into a table saw's rotating blade to fabricate a replacement part for dumper guides/rails when the plastic piece kicked back, causing the employee to momentarily lose control of it. His hands continued to move forward and his left middle fingertip contacted the rotating blade and was amputated. The blade was unguarded at the time of the incident.
