An employee was performing maintenance on a printing press. The press was locked/tagged out, and the employee disengaged and removed the folder mechanism. The employee then attempted to re-engage the folder mechanism and slid it back into the press. As he was re-engaging the mechanism, his left index finger was caught and partially amputated in the mechanism's gear and chain.
