An employee was inserting a power spreader beneath a feed truck while replacing the crossbar of the vehicle. As he attempted to place the tool in the proper position, the coupler on the tool failed and discharged the adapter, striking him in the face. He sustained a laceration on his face and a skull fracture, requiring hospitalization.
