Final narrative
An employee operated a forklift to transport a hopper outside in order to empty the hopper contents into a dumpster. The employee got off the forklift and tried to move down the manual lever that causes the hopper to empty, but the lever was tight. When the employee finally got the lever to go down, the hopper shifted and caught the employee's hand between the hopper and a forklift fork, resulting in the amputation of the employee's left middle finger at the first knuckle.
