An employee was riding on a pallet of flooring materials on a powered industrial truck (PIT) to stabilize the load. The PIT was being operated with the forks 4-6 feet high. While it traveled from inside the building to outside the building, the bottom of the pallet broke. The pallet flipped forward and the employee was thrown into the air and landed on the ground. The employee sustained a separated shoulder blade, fractured wrist and arm, torn ACLs in both knees that required surgery, and a chest contusion.

An employee was operating a forklift to move panels around the dock area when the vehicle struck a joint where the two floors meet.
