Final narrative
An employee was crimping the tops of flash bang grenades while they were on a lathe. The crimp is applied via a plunger that rolls the edges. The employee placed a flash bang grenade onto the lathe and engaged the jaws to hold it in place. While the employee was still holding the grenade, the pneumatic plunger engaged and struck the employee's index finger on his right hand, resulting in a finger amputation.
