An employee was on a 12-foot A-frame ladder inspecting an air leak on a discharge pipe. As the employee was descending the ladder, he accidentally opened a release valve, which released a powder-like substance onto his face; this caused him to lose balance and fall from the ladder to the ground. He suffered broken ribs.
