On May 29, 2019, at 1:30 p.m., an electrical maintenance technician was using a wrench to tighten a bolt on a roof exhaust fan assembly. As the employee was tightening the bolt, it broke causing their right hand to strike against the fan assembly resulting in amputation of the ring and index fingertips. The fan's energy source was isolated and locked out at the time.
