On January 21, 2020, an employee was assessing an air handler unit that needed new insulation when the employee's left hand came in contact with the belt and pulley on the unit, resulting in a fingertip amputation. The belt and pulley were not completely guarded, and the unit was not locked/tagged out at the time of the incident.
