On February 5, 2020, an employee was training a new employee on a notcher machine, which creates half moon-shaped notches on pallets. The employee changed the setting on the machine to create notches on 50-inch long lumber. The machine then jammed and he immediately pushed the E-stop button. He attempted to remove the jammed piece of wood with his left hand when the machine's blade caused an amputation of his left thumb tip and index fingertip with bone loss.
