On February 23, 2020, at approximately 12:30 a.m. an employee was working in a commercial bakery on a bread line machine where the dough is rolled into a loaf. The employee needed to clean the machine top belt, which is done by manually opening the top of the belt in a door-like fashion. The employee finished cleaning the belt and when he closed the belt manually, the employee's left pinky finger was closed in the belt and amputated.
