On 1/30/2021, at 6:00 AM, an employee was operating an overhead crane to move steel plate material in the horizontal position. The crane was equipped with lifting devices including a spreader bar, chains, and plate clamps. The rigging failed and the steel plate fell on the employee's right ankle resulting in lower leg fractures.
