An employee and a coworker were manually transferring a die set (593 pounds) from a maintenance table to an industrial cart. The cart consists of a stationary platform and a lift platform controlled by a chain and hydraulic cylinder operable by foot pump. After the two employees pulled the die set onto the raised lift platform from the table, the chain over the hydraulic cylinder came free from the threaded rod and nut that serve as an anchor point on the stationary platform, causing the lift platform with die set to fall from table height to the ground. The die set struck the injured employee's leg as it fell, resulting in lacerations and fractures to the shin and ankle.
