On April 16, 2021, at approximately 1:20 a.m., an employee was installing a new emergency/parking brake on a compact wheel loader equipped with a pallet fork attachment. While the employee was under the arms and pallet fork attachment of the wheel loader, the hydraulic cable was released causing hydraulic oil and pressure to escape. The arms and pallet fork attachment lowered and pinned the employee's left arm and leg between the pallet fork attachment and the ground. The employee's lower left arm and wrist were crushed. The employee also sustained broken bones in lower left leg and multiple broken vertebrae in lower back.
