An employee was using a step ladder to access a conveyor belt 10 feet above the floor. He lost his footing on the ladder, reached out to grab something stable, and grabbed the tension roller. The running belt pulled his right hand and forearm in between the tensioner and the belt, causing a compound fracture to the humerus and radius.
