An employee was using a forklift to pull down a pallet from a steel rack. A bulb fell from a light fixture and made contact with the forklift, startling the employee and resulting in the control handle getting pulled back, which caused the employee's left hand to get caught between the cabin and the steel racking. The employee's left little finger was amputated.
