Final narrative
An employee locked out a machine that stretches bread dough in order to replace the chain. After replacing the chain, the employee removed the lockout tag and was watching the chain cycle through. The employee leaned up against the equipment frame when his left hand slipped and went into the chain and sprocket, resulting in an amputation of the index finger.
