An employee noticed that plastic film was sticking to a die on a machine that cuts the film into boot shapes. The employee was using a tool to apply corn starch powder to the conveyor belt when the tool became lodged in an ingoing nip point. The nip point then caught the employee's right index finger and partially amputated the fingertip.
