On February 15, 2022, an employee was offloading 5 pieces of 1/2-inch bent plates that had a total weight of 1,225 pounds. The employee was using an overhead crane equipped with a new basket lift sling. Sharp edges on the bent plates cut through the lifting sling causing the plates to fall about 3 feet to the floor. One of the plates fell on the employee's right leg causing a laceration. The employee then developed a hematoma of his lower right leg and was hospitalized.
