An employee was repairing a leaking hose that fed acid into a mixer. After removing the hose from the mixer, he lost his balance, tried to catch himself, and caught his finger in the hose hole of the mixer as he fell to the ground. He suffered severe damage to his right middle finger from contact with the sharp sand inside the mixer hole. The fingertip was medically amputated.
