A mechanic was using a torch to heat a wheel bearing in order to remove the bearing. A shop rag caught fire. Another employee threw washer fluid on the burning rag. The washer fluid caused a flash fire and the mechanic sustained second-degree burns to the face, chest, and arms.

Employee 1 saw fire behind the welding smoke and notified employee 2. Before they could get off the tank, they both fell in the tank and sustained burns from the fire.
