Final narrative
An employee was unloading scrap tires from a forklift into a trailer. As the employee lowered the forklift mast to drop the tires, the tires became pinched between the forklift tines and the floor of the trailer. The employee then manually pushed the tires into the trailer and once free, the forklift tines lowered onto his left ring finger, resulting in a laceration and skin loss.

The employee slipped and as he went to catch himself, he accidentally engaged the up-down control for the forks. This caused the forks to fall onto his foot.
