An employee was using an electric pallet jack to move hand holes out of a delivery truck. One of the straps securing the double stacked hand holes broke, causing the top half of the stack to fall. The employee fell 3-4 feet off the delivery truck lift gate, resulting in a lower back fracture and brain hemorrhaging.
