At about 9:45 a.m. on May 25, 2023, an employee was installing a spreader pipe on a 10-by-10 manhole box. While the pipe was being moved into position, the spreader slipped. The employee's thumb was caught in an eyehole and pinched between the spreader and the receiver, causing an amputation to the thumb tip.
