An employee was using a raised forklift to pull a load from the third-level of a rack. The load knocked into a horizontal support beam on the rack, causing it to disconnect from the vertical column on the right side. The left side was still connected to the rack. After reattaching the beam, the forklift mast was lowered and the beam came off, causing the pallet to shift and drop a case. The box struck the employee's right Achilles tendon, causing soft tissue damage.
