An employee was backing up a powered industrial truck (PIT) when it collided with an elevator door. The collision caused the elevator doors to open, and the employee and PIT fell to the bottomo of the elevator shaft 6 to 7.5 feet below. The employee sustained fractures to the right leg, lumbar vertebra, and head injuries.
