Final narrative
An employee was transferring a chemical from a larger tote through a pumping system into a drum. When the chemical (a catalyst instead of the intended binder chemical) was pumped into the drum, a chemical reaction occurred and the barrel combusted. The employee suffered first-degree chemical burns to the face and second-degree chemical burns to the left forearm, back, and head.
