Final narrative
An employee was reversing a forklift while the welding table was on the forks. The welding table struck storage racks and was caught. When the employee went to free the table, it flipped upside down off the forks. The employee then lifted the table with the forklift again and exited the forklift to adjust the table manually. The table slipped and crushed the employee's right hand, amputating the middle finger between the first and second joint from the tip.
