Final narrative
An employee was using a service boom truck to assist in removing a bell crank on the front of a heavy duty pay loader. The employee attempted to unhook the cylinder from the bell crank with the boom truck when the lifting sling holding the cylinder moved, causing the employee's gloved fingers to be pinched between the bell crank and cylinder eye. The employee's middle and ring fingers were amputated at the first knuckle.

An employee was setting a rebuilt water pump using a forklift and a chain after inspecting the area where the pump was going to be placed. While the employee's right hand was on a nearby valve flange, the water pump shifted while in the air and struck the employee's right index finger, fracturing it. The employee was hospitalized and required surgery.

Employees were loading a 295-pound transformer onto a pickup truck using a forklift. The injured employee was acting as the spotter. The transformer hit the tailgate of the pickup truck and the spotter's right hand was struck by the transformer, resulting in amputation of the ring fingertip.

An employee was using a crane to load steel plates onto a trailer. As he raised the crane up, one side of the clamp was not unhooked, which pulled the steel plate over onto the employee. They suffered a crushed left leg and right ankle.
STERLING CITY, TEXAS—

An employee was working on a drilling rig, tripping in a stand of drill pipe. While the final stand was being set into the elevators, the pipe made contact with the latch mechanism rather than entering the elevator opening. This misalignment caused the stand to rebound toward the employee. The employee was stabilizing the pipe when it ricocheted back toward the elevators. The employee s left ring finger was caught and broken between the drill pipe and the elevator assembly.
