Two employees were using a stressing machine to pull and tension a cable to 3,000 pounds per square inch when the chuck assembly fell into the well of the hydraulic ram. They stopped stressing the cable and the injured employee retrieved the chuck and was positioning it in the well when the stressing machine was activated. The injured employee's right index and middle fingers were caught between the ram and the inside wall of the hydraulic ram, resulting in a partial index finger amputation and a fractured middle finger.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.
