On December 30, 2024, at approximately 8:00 PM, an employee was troubleshooting a packaging machine that was jammed with plastic. When the employee went to pull the plastic out from the vacuum seal section, the machine actuated and his right middle and ring fingers were caught in the machine. The employee sustained partial amputations to both fingers above the first knuckles. The machine was energized and not locked out/tagged out at the time.
