An employee was spraying bearings on a laminator. The laminator had been covered with a rag to protect it from overspray; the rag was pulled into the laminator's gears, also pulling in the employee's left hand. The gears crushed and lacerated the employee's left index, middle, ring, and little fingers, resulting in the amputation of all four fingers.
