An employee was removing a tube that was stuck in a draw bench. The tube activated a sensor that dropped the inserter down, causing the tube to pull the employee's left little finger into the tube and the inserter wheels. The employee sustained a partial amputation and was hospitalized. The machine was not locked out at the time.
