Employees were installing decking on a low-slope roof. One of them slipped and fell through a hole onto the concrete floor 22 feet below. The employee suffered fractures to their collarbone, ribs, and pelvis and was hospitalized. Fall protection was not attached at the time of the incident.

The employee's wrench slipped off the equipment, causing them to lose their balance and fall.
