A forklift operator was preparing to unload a semi-truck trailer. The forklift entered the trailer and picked up a pallet. The forklift then reversed and the trailer started rolling forward. The forklift fell from the trailer to the ground below. The forklift operator was hospitalized with head and arm pain. Wheel chocks had not been used under the rear wheels of the semi-truck.
